Due to the need to conserve surgical supplies and equipment, both on the human and animal side, spay/neuter surgery is not considered an emergency function. Consequently, you are bound by this agreement to return the animal mentioned above in 60 days or not later than the date noted below for spay/neuter of this pet. An appointment must be made in advance by calling 407-742-8000 and advising the receptionist that you need to schedule spay/neuter of a pet that was adopted from us. Please keep track of your adoption paperwork as we use the Animal ID number to track information. In lieu of bringing the pet back to us for spay/neuter, you have the option of having it done by your veterinarian, at your expense, and there will be no refund from us. In this case, you will need to provide proof of spay/neuter within the 60 day time frame. Proof of surgery must be on a veterinary letterhead and signed by the veterinarian along with a complete description of the pet and its ID number. If you have any questions about our ability to perform the surgery by the deadline due to ongoing issues with the pandemic, please call us at 407-742-8000. By signing below, I am agreeing to the stipulations detailed above. I am agreeing to these stipulations of my own free will and am not under duress nor have I been coerced. It shall be a violation of the Osceola County Animal Code, Chapter 4, Article 1, Section 4-4 (c), for any animal owner entering into a stipulated agreement to fail to comply, in whole or part, with the provisions of the agreement. 60 days for spay/neuter ends onMonthMonth123456789101112DayDay12345678910111213141516171819202122232425262728293031YearYear2025202420232022202120202019201820172016201520142013201220112010200920082007200620052004200320022001200019991998199719961995199419931992199119901989198819871986198519841983198219811980197919781977197619751974197319721971197019691968196719661965196419631962196119601959195819571956195519541953195219511950194919481947194619451944194319421941194019391938193719361935193419331932193119301929192819271926192519241923192219211920Animal id number (preceded by an A)SignatureCommentsThis field is for validation purposes and should be left unchanged.
